MACK 4 YEAR DIGITAL VIDEO WARRANTY

4-DV XXXX  LIMITED WARRANTY OF REPAIR

This warranty is an extension of the manufacturer's normal warranty on the
equipment for an additional period of 4 year. The digital video equipment
registered below is guaranteed to operate properly according to
manufacturer's specifications or we shall repair to operate properly.

In the event that this equipement requires any repairs or service, it must
be delivered post paid with warranty #, the owner's name and address, proof
of purchase and description of difficulties encountered to Mack Camera &
Video Service, 200 Morris Ave., Springfield, NJ 07081 with a check for $15
for shipping and handling in the continental U.S. (cost for overseas
shipping to be determined) Call (973) 467-2291 or 800-252-MACK (outside of
NJ) for a U.P.S. Call Tag to be issued or for a Mack authorized service
center near you (at the discretion of Mack Camera). Servicer must have
written authorization from Mack Camera before any repairs are completed.

The equipment will be restored to proper operating condition at no charge
to you (this includes warranted parts and labor and head replacement when
necessary.) Such service, repair or adjustment of the equipment is assured
the original purchaser only. This warranty does not cover equipment which
is misused, tampered with, modified or damaged as a result of accident,
water, sand, impact or lack of proper care as indicated in the manual of
operations. Accessories not covered: cable boxes, connectors, screens,
LCDs, lenses, antennas, decorative parts, A/C adapters, cables, handles,
carry straps, knobs, buttons, remote, routine maintenance, rusting,
corrosion, or batteries. Not for professional use. This limited warranty is
not transferrable. Mack Camera & Video Service reserves the right to
terminate warranty contracts if invoices are not paid according to terms,
and not valid until after receipt of payment from dealer. One head cleaning
per year at Mack Camera.

Retain this portion of your contract, it is your proof of ownership. You
will be required to present it for service. This limited warranty applies
only to the product specified herein. It does not cover loss of tapes or
other expenses or consequential damages.
